About Us

CPACS/Cosmo is a Community Health Center dedicated to providing excellent primary care services. Community Health Centers (or Federally Qualified Health Centers) are non-profit, community-owned health centers which operate in areas where care is needed but availability is low.

Our center is affiliated with the Center for Pan Asian Community Services (CPACS), one of the largest social services organizations in the Southeast with the capacity to serve immigrants, refugees, and underserved communities. While we have been providing community outreach health services since 1980, in 2013 we developed our independent health center to provide comprehensive primary care. We also link our patients to useful social services through this partnership.
